 We created the Nail Laser Centers specifically to deal with the treatment of an old problem with new and exciting technology.  We are podiatrists, foot specialists, and have been treating nail fungus for well over 25 years. In the past we have been as frustrated as you with the limited and unsuccessful treatment options available to deal with this difficult problem. Lasers are very new in treating nail fungus and we have been searching for well over a year to find the right laser. We believe we have now found the most sophisticated laser on the market with the power and versatility to offer our patients a new option in the treatment of nail fungus.

The Problem


Toenail fungus (onychomycosis) is an infection of the nail and skin beneath the nail that affects nearly 23 million people in the US-about 10% of all adults.  It is a difficult infection to treat. To date treatments included topical medications which don't work because it is nearly impossible for the medication to penetrate the nail in order for it to reach the infection. Oral medications are available, however they also have limited success (60-70%), must be taken for at least 3 months, and have the potential to cause serious side effects i.e. liver complications and drug interactions. Additionally oral medications only treat a limited number of the over 200 fungi that can cause this infection. The newest treatment available uses laser technology to actually kill the fungus at the source of the infection.


The Solution


There is no perfect treatment for nail fungus, however the latest studies using lasers have shown a success rate of 80-90%. Using one of the most advanced lasers available, near infrared laser light passes through the nail killing the fungus while leaving the nail and surrounding soft tissue unharmed.


Unlike oral medications, the Nail Laser has the unique ability to attack the more than 200 fungi that can be responsible for causing this infection.  The laser we are currently using in our laser centers gives us an advantage over other lasers in that we have the option to change numerous settings on the laser to provide you the best treatment outcome.  All our patients are evaluated and treated by licensed physicians. Unlike other single laser treatments, we offer a TWO treatment protocol spaced four months apart to achieve the best outcome.

 Plantar fasciitis is the term commonly used to refer to heel and arch pain traced to an inflammation on the bottom of the foot. More specifically, plantar fasciitis is an inflammation of the connective tissue, called plantar fascia, that stretches from the base of the toes, across the arch of the foot, to the point at which it inserts into the heel bone. Overpronation is the most common cause of plantar fasciitis. As the foot rolls inward excessively when walking, it flattens the foot, lengthens the arch, and puts added tension on the plantar fascia. Over time, this causes inflammation.

Also known as heel spur syndrome, the condition is often successfully treated with conservative measures, such as the use of anti-inflammatory medications, ice packs, stretching exercises, orthotic devices, and physical therapy. Note: Please consult your physician before taking any medications. In persistent cases, Extracorporeal Shock Wave Treatment (ESWT) may be used to treat the heel pain.

 Heel pain is an extremely common and potentially disruptive affliction that has many possible causes, including stress fractures, arthritis, nerve irritation, cysts, tendonitis and most often, plantar fasciitis. Plantar fasciitis occurs when the tissue that connects the heel to the toes becomes inflamed. This inflammation causes pain on the bottom of the heel that can continue to grow in intensity over time.

You may be prone to developing this painful condition if the mechanics of your feet aren’t ideal, such as flat feet or excessively high arches, or if you have poor alignment in your feet that causes you to walk on the inner or outer edges of your feet. Overweight individuals may be more likely to develop plantar fasciitis and it can occur from spending hours on the feet in shoes that offer little or no support.

Treatment options for plantar fasciitis included an effective stretching regimen, icing, footwear modifications, anti-inflammatory medications and weigh reduction to lessen the impact on the feet. More severe cases may be treated with additional padding and orthotic devices, the use of a walking cast, night splints and physical therapy. Proper footwear is an important part of an overall treatment program for people with diabetes, even at the earliest stages of the disease. If there is any evidence of neuropathy, wearing the right footwear is crucial.

As a general rule, people with diabetes should choose shoes that:

  • Accommodate, stabilize, and support deformities, such as Charcot Foot, loss of fatty tissue, hammertoes, and amputations. Many deformities need to be stabilized to relieve pain and avoid further damage. In addition, some deformities may need to be controlled or supported to decrease further progression of the deformity.
  • Limit motion of joints. Limiting the motion of certain joints in the foot can decrease inflammation, relieve pain, and result in a more stable and functional foot.
  • Reduce shock and shear. A reduction in the overall amount of vertical pressure, or shock, on the bottom of the foot is desirable, as well as a reduction of horizontal movement of the foot within the shoe, or shear.
  • Relieve areas of excessive pressure. Any area where there is excessive pressure on the foot can lead to skin breakdown or ulcers. Footwear should help to relieve these high pressure areas, and therefore reduce the occurrence of related problems.

Prescription Footwear

Many diabetics need special prescription footwear. The various types include:

  • Custom-made shoes. When extremely severe deformities are present, a custom-made shoe can be constructed from a cast or model of the patient's foot. With extensive modifications of in-depth shoes, even the most severe deformities can usually be accommodated.
  • External shoe modifications. In these cases, the outside of the shoe is modified in some way, such as adjusting the shape of the sole or adding shock-absorbing or stabilizing materials.
  • Healing shoes. Immediately following surgery or ulcer treatment, special shoes may be necessary before a regular shoe can be worn. These include custom sandals (open toe), heat-moldable healing shoes (closed toe), and post-operative shoes.
  • In-depth shoes. An in-depth shoe is the basis for most footwear prescriptions. It is generally an oxford-type or athletic shoe with an additional 1/4-inch to 1/2-inch of depth throughout the shoe. This extra volume accommodates inserts, or orthotics, as well as deformities commonly associated with a diabetic foot. In-depth shoes are usually designed to be light in weight, have shock-absorbing soles, and come in a wide range of shapes and sizes to accommodate virtually any foot.
  • Orthoses or shoe inserts. Also known as orthotics, an orthosis is a removable insole which provides pressure relief and shock absorption. Both pre-made and custom-made orthotics or shoe inserts are commonly recommended for patients with diabetes, including a special total contact orthosis, which is made from a model of the patient's foot and offers a high level of comfort and pressure relief.

Proper footwear is an important part of an overall treatment program for people with diabetes, even at the earliest stages of the disease. If there is any evidence of neuropathy, wearing the right footwear is crucial.

As a general rule, people with diabetes should choose shoes that:

  • Accommodate, stabilize, and support deformities, such as Charcot Foot, loss of fatty tissue, hammertoes, and amputations. Many deformities need to be stabilized to relieve pain and avoid further damage. In addition, some deformities may need to be controlled or supported to decrease further progression of the deformity.
  • Limit motion of joints. Limiting the motion of certain joints in the foot can decrease inflammation, relieve pain, and result in a more stable and functional foot.
  • Reduce shock and shear. A reduction in the overall amount of vertical pressure, or shock, on the bottom of the foot is desirable, as well as a reduction of horizontal movement of the foot within the shoe, or shear.
  • Relieve areas of excessive pressure. Any area where there is excessive pressure on the foot can lead to skin breakdown or ulcers. Footwear should help to relieve these high pressure areas, and therefore reduce the occurrence of related problems.

Prescription Footwear

Many diabetics need special prescription footwear. The various types include:

  • Custom-made shoes. When extremely severe deformities are present, a custom-made shoe can be constructed from a cast or model of the patient's foot. With extensive modifications of in-depth shoes, even the most severe deformities can usually be accommodated.
  • External shoe modifications. In these cases, the outside of the shoe is modified in some way, such as adjusting the shape of the sole or adding shock-absorbing or stabilizing materials.
  • Healing shoes. Immediately following surgery or ulcer treatment, special shoes may be necessary before a regular shoe can be worn. These include custom sandals (open toe), heat-moldable healing shoes (closed toe), and post-operative shoes.
  • In-depth shoes. An in-depth shoe is the basis for most footwear prescriptions. It is generally an oxford-type or athletic shoe with an additional 1/4-inch to 1/2-inch of depth throughout the shoe. This extra volume accommodates inserts, or orthotics, as well as deformities commonly associated with a diabetic foot. In-depth shoes are usually designed to be light in weight, have shock-absorbing soles, and come in a wide range of shapes and sizes to accommodate virtually any foot.
  • Orthoses or shoe inserts. Also known as orthotics, an orthosis is a removable insole which provides pressure relief and shock absorption. Both pre-made and custom-made orthotics or shoe inserts are commonly recommended for patients with diabetes, including a special total contact orthosis, which is made from a model of the patient's foot and offers a high level of comfort and pressure relief.

 According to the American Diabetes Association, about 15.7 million people (5.9 percent of the United States population) have diabetes. Nervous system damage (also called neuropathy) affects about 60 to 70 percent of people with diabetes and is a major complication that may cause diabetics to lose feeling in their feet or hands.

Foot problems are a big risk in diabetics. Diabetics must constantly monitor their feet or face severe consequences, including amputation.

With a diabetic foot, a wound as small as a blister from wearing a shoe that's too tight can cause a lot of damage. Diabetes decreases blood flow, so injuries are slow to heal. When your wound is not healing, it's at risk for infection. As a diabetic, your infections spread quickly. If you have diabetes, you should inspect your feet every day. Look for puncture wounds, bruises, pressure areas, redness, warmth, blisters, ulcers, scratches, cuts and nail problems. Get someone to help you, or use a mirror.

  • Always keep your feet warm.
  • Don't get your feet wet in snow or rain.
  • Don't put your feet on radiators or in front of the fireplace.
  • Don't smoke or sit cross-legged. Both decrease blood supply to your feet.
  • Don't soak your feet.
  • Don't use antiseptic solutions, drugstore medications, heating pads or sharp instruments on your feet.
  • Trim your toenails straight across. Avoid cutting the corners. Use a nail file or emery board. If you find an ingrown toenail, contact our office.
  • Use quality lotion to keep the skin of your feet soft and moist, but don't put any lotion between your toes.
  • Wash your feet every day with mild soap and warm water.
  • Wear loose socks to bed.
  • Wear warm socks and shoes in winter.
  • When drying your feet, pat each foot with a towel and be careful between your toes.
  • Buy shoes that are comfortable without a "breaking in" period. Check how your shoe fits in width, length, back, bottom of heel, and sole. Avoid pointed-toe styles and high heels. Try to get shoes made with leather upper material and deep toe boxes. Wear new shoes for only two hours or less at a time. Don't wear the same pair every day. Inspect the inside of each shoe before putting it on. Don't lace your shoes too tightly or loosely.
  • Choose socks and stockings carefully. Wear clean, dry socks every day. Avoid socks with holes or wrinkles. Thin cotton socks are more absorbent for summer wear. Square-toes socks will not squeeze your toes. Avoid stockings with elastic tops.

Heel Pain

In adults, heel pain is a common condition most often caused by straining or tearing the arch ligament where it attaches to the heel. This is known as plantar fascitis.

Pain is felt under the heel when walking and is typically worst when getting up after resting. The heel usually feels bruised. If sharp pains are present, a nerve may be involved. Heel spurs are often associated with this condition and are diagnosed by x-ray examination. A heel spur is a small projection of bone that forms where the ligament has been tearing from its connection to the heel. The spur is not the source of the heel pain and does not need to be removed.

Treatment is aimed at stopping the arch ligament from tearing, as this is the source of the heel pain. Proper foot support is used to accomplish this. Custom made foot supports, known as orthotics, provide the best support and are the most comfortable. To make orthotics, Dr. Dalvin takes plaster molds of the feet, writes a prescription, and sends the molds to a laboratory. Non-prescription arch supports are available at sporting goods stores and in the foot care section of local pharmacies.

Additional treatment for heel pain may include injections of cortisone, oral anti-inflammatory medication, and stretching exercises. When pain is difficult to resolve, blood tests may be recommended and additional treatment is available with night splints, Aircasts, shockwave therapy (click on the link to the left for additonal information), and surgery as a last resort.

Surgery is only needed in a small percentage of cases, but when surgery is needed, Dr. Dalvin uses an endoscopic procedure. The endoscopic procedure involves two tiny incisions and allows for the fastest recovery.
