Plantar fasciitis is the term commonly used to refer to heel and arch pain traced to an inflammation on the bottom of the foot. More specifically, plantar fasciitis is an inflammation of the connective tissue, called plantar fascia, that stretches from the base of the toes, across the arch of the foot, to the point at which it inserts into the heel bone. Overpronation is the most common cause of plantar fasciitis. As the foot rolls inward excessively when walking, it flattens the foot, lengthens the arch, and puts added tension on the plantar fascia. Over time, this causes inflammation.

Also known as heel spur syndrome, the condition is often successfully treated with conservative measures, such as the use of anti-inflammatory medications, ice packs, stretching exercises, orthotic devices, and physical therapy. Note: Please consult your physician before taking any medications. In persistent cases, Extracorporeal Shock Wave Treatment (ESWT) may be used to treat the heel pain.

A neuroma is an enlarged, benign growth of nerves, most commonly between the third and fourth toes. Neuromas are caused by tissue rubbing against and irritating the nerves. Pressure from poorly fitting shoes or an abnormal bone structure can also lead to this condition. Symptoms may include sensations of thickness, burning, numbness, tingling, or pain in the ball of the foot. Treatments generally include wearing corrective shoes or orthotics and/or cortisone injections. In severe cases, surgical removal of the growth may be necessary.

Morton's neuroma is a thickening of tissues around the nerve that leads to the toes. Morton's neuroma usually develops between the third and fourth toes in response to irritation, such as that caused by wearing high-heeled or narrow shoes, or from trauma. Symptoms may include a burning pain that radiates from the ball of the foot to the toes or numbness in the toes. Conservative treatments usually resolve the pain or progressions of the condition, and range from wearing roomier, lower-heeled footwear or using orthotics to reduce the pressure on the nerve, to injections of cortosteroid medication to reduce swelling and inflammation.

A bunion is a bone deformity caused by an enlargement of the joint at the base and side of the big toe (metatarsophalangeal joint). Bunions form when the toe moves out of place. The enlargement and its protuberance cause friction and pressure as they rub against footwear. Over time, the movement of the big toe angles in toward the other toes, sometimes overlapping a third toe (known as Hallux Valgus). The growing enlargement or protuberance then causes more irritation or inflammation. In some cases, the big toe moves toward the second toe and rotates or twists, which is known as Hallus Abducto Valgus. Bunions can also lead to other toe deformities, such as hammertoe.

Many people with bunions suffer from discomfort and pain from the constant irritation, rubbing, and friction of the enlargement against shoes. The skin over the toe becomes red and tender. Because this joint flexes with every step, the bigger the bunion gets, the more it hurts to walk. Over time, bursitis or arthritis may set in, the skin on the bottom of the foot may become thicker, and everyday walking may become difficult—all contributing to chronic pain.

Wearing shoes that are too tight is the leading cause of bunions. Bunions are not hereditary, but they do tend to run in families, usually because of a faulty foot structure. Foot injuries, neuromuscular problems, flat feet, and pronated feet can contribute to their formation. It is estimated that bunions occur in 33 percent of the population in Western countries.

Depending on the size of the enlargement, misalignment of the toe, and pain experienced, conservative treatments may not be adequate to prevent progressive damage from bunions. In these cases, bunion surgery, known as a bunionectomy, may be advised to remove the bunion and realign the toe.

Dr. Tracey G. Toback is one of the first Board Certified Foot Surgeons in the Hudson Valley, and a Diplomate of the American Board of Podiatric Foot Surgery. He has been practicing podiatry in the Hudson Valley since 1986, offering the highest level of comprehensive foot care.

A graduate of the New York College of Podiatric Medicine and Certified by the American Board of Podiatric Surgery, Dr. Toback also served as Assistant Professor for the Department of Medicine and Radiology at the New York College of Podiatric Medicine and Foot Clinics of New York in Manhattan. He is a Fellow of the American College of Podiatric Radiologists, the American Society of Podiatric Dermatologists and the American College of Foot and Ankle Surgeons.

Dr. Toback is a published author and regularly gives guest lectures. He is continually refining his podiatric expertise by attending several educational conferences and seminars annually, whereby offering his patients the newest and most technologically advanced procedures and treatment available.

The heel bone (calcaneus) is the largest bone in the foot. This strong bone supports the body's total weight and is responsible for absorbing the daily stress placed on the feet, making it susceptible to various heel pain and disorders.

Heel pain is also associated with an important ligament of the foot called the plantar fascia. The plantar fascia is attached to the bottom of the heel and extends up to the toes to create and support the arch of the foot. Should the arch flatten due to stress, the plantar fascia stretches beyond its limit (sometimes tearing or rupturing) and pulls on the heel, causing great discomfort or a condition known as plantar fasciitis.

Patients with heel pain often describe the pain as “stepping on a spur or thorn", thus the name heel spur. X-rays are often taken and reveal a "bony spur" that has formed at the point where the calcaneus attaches to the plantar fascia. Every step becomes a painful experience and surgical intervention is often necessary to remove the spur and alleviate the pain. Additional physical factors that contribute to heel pain include flat feet, high arches, and tight calf muscles. Significant weight gain, high impact athletic activities, and extended periods of walking or standing can also cause heel pain to flare up.

Several treatment options are recommended for heel pain, including steroid or cortisone injections, anti-inflammatory medications, heel cups and cushions, and orthotic therapy. Soaking in warm water with Epsom salts provides temporary relief. Your podiatrist may also recommend some exercises to help stretch the calf muscles. In severe cases, surgical removal of the heel spur is the only option.

When conservative treatments fail to offer relief, Dr. Toback may perform a procedure called an endoscopic plantar fasciotomy. During this procedure, a small instrument called an arthroscope is inserted into the foot to surgically release the tight tendon where it attaches to the heel bone. Two small incisions are made on either side of the heel. Through one opening, the arthroscope is attached to a small camera to allow the podiatrist to see the inflamed tendon on a TV. Monitor. Through the other opening, small tools are utilized to make a tiny cut in the tendon, thus releasing the tension by allowing the tendon to lengthen, furthermore relieving the tension on the heel spur. The actual heel spur is not removed.

Endoscopic surgery is the newest and best course of treatment. The incisions are very small and consequently, there is less invasive work and discomfort. The procedure is performed in an outpatient surgical center and no hospital stay is required. The procedure itself takes about 15 minutes and the patient will begin walking immediately. Minimal loss of work is incurred.

Older, traditional methods of heel spur surgery required a large surgical incision across the inner side of the heel and a hospital stay was required. A painful recovery generally lasted 4-6 weeks, making it very difficult to return to work. Endoscopic surgery is truly one of the many great advances made in modern medicine.

Various types of fungi are present everywhere in the environment. The dark, moist surroundings created by shoes and stockings make the feet especially susceptible to fungal infection.

Most fungi are harmless until they penetrate the skin. A fungus can invade through minor cuts, or after injury or repeated irritation to the toes have caused the nail to separate from the bed. Fungal infections of the nail plate and nail matrix are quite common.

Symptoms

•Fungus may cause the nail to thicken and become yellow or brownish.

•As the fungus grows, foul-smelling, moist debris can be seen.

•Pressure from a thickened nail or the build-up of debris may make the toe painful.

Treatments

Treatment is best begun at the early stages of infection. The accumulation of debris under the nail plate can lead to an ingrown nail, or to a more serious bacterial infection that can spread beyond the foot.

To reduce pain associated with a thickened, infected nail, your doctor may reduce its thickness by filing the nail plate down with a burr. Filing will not, however, prevent the infection from spreading.

Oral and/or topical medications may be prescribed when:

•Only a small portion of one nail is infected

•Several nails are affected

•Keeping the nail is desired

Medication may or may not completely eliminate the fungus. Often, after medication is discontinued, the fungus recurs. Your podiatric surgeon will monitor the results of oral prescriptions carefully, and will explain any possible side effects.

While topical medications usually do not eliminate the fungus, they may be effective when used directly on the nail bed, after the nail plate has been removed.

A bunion is a bone deformity caused by an enlargement of the joint at the base and side of the big toe (metatarsophalangeal joint). Bunions form when the toe moves out of place. The enlargement and its protuberance cause friction and pressure as they rub against footwear. Over time, the movement of the big toe angles in toward the other toes, sometimes overlapping a third toe (known as Hallux Valgus). The growing enlargement or protuberance then causes more irritation or inflammation. In some cases, the big toe moves toward the second toe and rotates or twists, which is known as Hallus Abducto Valgus. Bunions can also lead to other toe deformities, such as hammertoe.

Many people with bunions suffer from discomfort and pain from the constant irritation, rubbing, and friction of the enlargement against shoes. The skin over the toe becomes red and tender. Because this joint flexes with every step, the bigger the bunion gets, the more it hurts to walk. Over time, bursitis or arthritis may set in, the skin on the bottom of the foot may become thicker, and everyday walking may become difficult—all contributing to chronic pain.

Wearing shoes that are too tight is the leading cause of bunions. Bunions are not hereditary, but they do tend to run in families, usually because of a faulty foot structure. Foot injuries, neuromuscular problems, flat feet, and pronated feet can contribute to their formation. It is estimated that bunions occur in 33 percent of the population in Western countries.

Flat feet are a common condition of the foot structure. In infants and toddlers, prior to walking, the longitudinal arch is not developed and flat feet are normal. Most feet are flexible and an arch appears when children begin standing on their toes. The arch continues to develop throughout childhood, and by adulthood most people have developed normal arches.

Flat feet are generally associated with pronation, a leaning inward of the ankle bones toward the center line. Shoes of children who pronate, when placed side by side, will lean toward each other (after they have been worn long enough for the foot position to remodel their shape).

Many people with flat feet do not experience pain or other problems. When pain in the foot, ankle, or lower leg does occur, especially in children, the feet should be evaluated.

The goal for bunion treatment is twofold: first, to relieve the pressure and pain cause by irritations, and second to stop any progressive growth of the enlargement. Commonly used methods for reducing pressure and pain caused by bunions include:

•The use of protective padding, often made from felt material, to eliminate the friction against shoes and help alleviate inflammation and skin problems.

•Removal of corns and calluses on the foot.

•Changing to carefully fitted footwear designed to accommodate the bunion and not contribute toward its growth.

•Orthotic devices—both over-the-counter and custom made—to help stabilize the joint and place the foot in the correct position for walking and standing.

•Exercises to maintain joint mobility and prevent stiffness or arthritis.

•Splints for nighttime wear to help the toes and joint align properly. This is often recommended for adolescents with bunions, because their bone development may still be adaptable.

Surgical Treatment

Depending on the size of the enlargement, misalignment of the toe, and pain experienced, conservative treatments may not be adequate to prevent progressive damage from bunions. In these cases, bunion surgery, known as a bunionectomy, may be advised to remove the bunion and realign the toe.

Ankle sprains are caused by an unnatural twisting or force on the ankle bones of the foot, which may result in excessive stretching or tearing of one or more ligaments on the outside of the ankle. The severity of the sprain can impact the degree of damage as well as the type and duration of treatment. If not properly treated, ankle sprains may develop into long-term problems.

Primary symptoms of ankle sprains are pain following a twist or injury, swelling, and bruising.

Treatment includes resting and elevating the ankle and applying ice to reduce swelling. Compressive bandages also may be used to immobilize and support the injury during healing. Serious ankle sprains, particularly among competitive athletes, may require surgery to repair and tighten the damaged ligaments.

To prevent ankle sprains, try to maintain strength, balance, and flexibility in the foot and ankle through exercise and stretching, and wearing well-fitted shoes.

Well, basically I felt like I would have like a tendency to possibly fall or trip and lose my balance and it was numbness in my feet. I've had neuropathy of the legs and feet and partially even in in my fingers for the last 10 years if not longer. And one of the biggest problems I had was losing sleep because my toes bothered me. And you get a cramping feeling and eventually I'd get out of bed and walk around and try and walk it off and go back to sleep. But this can happen more than once or twice a night. In fact sometimes it lasts for many hours. And since I tried Neuremedy, I find that I'm not being woken up by the pain in my toes, and I seem to be getting some feeling back in my toes after an accident. And, you know, hopefully I'll get back to a lifestyle where I can live with the neuropathy. I want to get back to tennis. I want to get back to baseball. I want to get back to all the sports I used to be in and go from there. But the Neremedy seems to be working on me. I can't say it went away all the way, but it's gone away a lot. And I feel comfortable sleeping. And I haven't been woken up. I still have neuropathy, but I also am getting some feeling back. I was diagnosed at the age of 7 as an insulin dependent type 1 diabetic. So for 38 years I have been living the diet, exercise plan, and doing the best I can, but complications do happen. And about four years ago I began experiencing the results of peripheral neuropathy, pain, tingling, numbness in my feet.
